Formspree Form Processing

Our public contact forms may use Formspree to process form submissions and deliver messages to the email address connected with the form.

No PHI Through Public Forms

Please do not submit patient names, insurance IDs, medical record numbers, claim screenshots, medical details, or other protected health information through public website forms.

Business Associate Agreement

If a service relationship requires access to protected health information, a Business Associate Agreement and approved workflow should be completed during onboarding before PHI is shared.
